I understand that the $150 fee, plus any compact discs or tapes, are being provided to LAmusic.com in return for promotional web pages on LAmusic.com for a period of 1 year from the date the page is first available on the Internet. LAmusic.com's service includes, but is not limited to, regular updates of the artist/group's performance schedule. I further understand that the fee paid to LAmusic.com is non- refundable once the artist/group page is created, and LAmusic.com continues to provide service as described in the Terms of Service (see below). The artist/group retains all rights and copyrights to the text, images and sounds provided for presentation on LAmusic.com.

I authorize LAmusic.com to create an internet web page through which my products can be purchased by credit card holders. Along with this form, please provide LAmusic.com with:
- A copy of the CD(s) or tape(s) from which music samples are to be recorded. If the recording is on vinyl, please transfer to cassette or CD-R. CDs and tapes will not be returned.
- Indicate which tracks you wish to present on LAmusic.com.
- A song from each artist/group is featured in its entirety on the Bonus Cut page on a rotating basis. Indicate which entire song you would like to present on the Bonus Cut page.
- Complete lyrics of the songs to be presented on LAmusic.com.
- Any photos or artwork you would like to appear on your page. The number of images you can include will be determined by their size on the web page.
- A biography of the artist or group.
- Any reviews or other promotional material you would like to present on the page.
- A list of upcoming performances that includes venues, times, cover charges, etc. Your individual performance page is updated on an ongoing basis, so keep LAmusic.com posted on your gigs!
- Information on how to order the artist/group's recordings, or to join their mailing list.
- A check or money order for $150 made payable to LAmusic.com.
NOTE: Please provide all text to be included on the page via email. Format can be either Macintosh or PC, in Microsoft Word, WordPerfect, Simpletext, RTF or simple ASCII.

Terms of Service Provided by LAmusic.com
SERVICE PROVIDED:
For a fee of $150 for 1 year, payable in advance, LAmusic.com will provide:
- A home page on the Internet site located in the domain lamusic.com, created using text, images and recordings provided by the artist/group. The page will be created within 10 business days of the receipt of materials by LAmusic.com.
- LAmusic.com relies on information provided by the artist/group to keep the Upcoming Performances page current. Updates of this page will be made within 48 hours of receipt of information by LAmusic.com.
- LAmusic.com will provide a means for the artist/group to receive electronic mail (Email). If the artist/group has an existing mail address, a link will be provided to it. If the artist/group does not have an Email address, mail received for the artist/group will be forwarded by fax. Mail forwarded in this fashion is not private.
ARTIST RETAINS ALL RIGHTS
The artist/group retains all rights and copyrights to images, music, lyrics and other text provided to LAmusic.com, independent of the computer files generated to present them on the Internet.
CHANGES AND CORRECTIONS
Once the home page is created, the artist/group will have the opportunity to review and specify corrections and minor changes to their home page, which will be implemented within 1 week by LAmusic.com. Once the home page has been created, the $150 fee is not refundable. Any changes to, or updates of, the home page within the service period may or may not incur additional charges. No additional fees will be charged without the prior approval of the artist/group.
EXCLUSIVE USE
Computer files, including web pages, images and sound files generated for presentation on LAmusic.com are the exclusive property of LAmusic.com, and may not be used for any other purpose without the written consent of LAmusic.com. |
